HOW TO SIGN UP?

If you are a new registrant for ODP (after Nov. 6 and have not mailed in all the items) and planning to attend the Saturday, November 14 TRYOUT in Emporia, you MUST bring with you the Medical Information Form (notarized), registration fee, front/back copy of health insurance card and a small passport sized photo to the CHECK-IN prior to participation. If you are missing any of these, the player will not be allowed to participate in the tryout.

To now register manually, PRINT OFF a registration form. You MUST bring to check-in at the November 14 TRYOUT:
1) the completed registration form,
2)
registration fee,
3)
front/back copy of health insurance card and
4)
a small passport sized photo.

If you are unable to register online, PRINT OFF a registration form for the 2009-2010 Olympic Development Program. Please mail the completed registration form, registration fee, front/back copy of health insurance card and a small passport sized photo to KSYSA 708 S. Rogers Rd., STE. C, Olathe, KS 66062
You will be guided through the Registration process and notified of other items required for sign-up (Medical Release, copy of insurance card front/back and passport sized photo).
If you are unable to register online, you may request a Registration form and mail the completed form to KSYSA. If you need a Registration packet mailed to you, please contact the KSYSA office at (913)782-6434.
In order for a player to be registered for the 2009-2010 Kansas ODP program, the completed Registration Form (either via online registration or in hard copy), a small passport size photo, a copy (front & back) of your insurance card, the Medical Release, and full payment must be received at the KSYSA office, 708 S. Rogers Rd., Ste. C, Olathe, KS 66062 (different deadlines apply to different ages - see 2009-2010 Program Information for specifics).
